And as they did eat, Jesus took bread, and blessed and broke and gave to them, and said, Take it, and eat. Behold, this is for you to do in remembrance of my body; for as oft as you do this, you will remember this hour that I was with you. And he took the cup, and when he had given thanks, he gave it to them and they all drank of it. And he said unto them, This is in remembrance of my blood, which is shed for many, and the new covenant which I give unto you; for of me you shall bear record unto all the world, and as oft as you do this ordinance, you will remember me in this hour that I was with you, and drank with you of this cup, even the last time in my ministry. Truly I say unto you, of this you shall bear record, for I will no more drink of the fruit of the vine with you until that day that I drink it new in the kingdom of God. And now they were grieved, and wept over him.

The manner of their elders and priests administering the flesh and blood of Christ unto the church. And they administered it according to the commandments of Christ; wherefore, we know that the manner to be true. And the elder or priest did minister it; and they did kneel down with the church and pray to the Father in the name of Christ, saying, O God the Eternal Father, we ask thee in the name of thy Son Jesus Christ to bless and sanctify this bread to the souls of all those who partake of it, that they may eat in remembrance of the body of thy Son, and witness unto thee, O God the Eternal Father, that they are willing to take upon them the name of thy Son, and always remember him, and keep his commandments which he hath given them, that they may always have his spirit to be with them. Amen.
Moroni 4:1

The manner of administering the wine. Behold, they took the cup and said, O God the Eternal Father, we ask thee in the name of thy Son Jesus Christ to bless and sanctify this wine to the souls of all those who drink of it, that they may do it in remembrance of the blood of thy Son, which was shed for them, that they may witness unto thee, O God the Eternal Father, that they do always remember him, that they may have his spirit to be with them. Amen.
